      In addition to completing this application, please send the following information to the address below:

      • a transcript from the last high school you attended (or GED certificate),
      • any post-secondary transcripts (if applicable), and
      • an ACT score (if available)

      Some programs may request additional information or testing. Your file will not be complete until this information is received.
